RELEASE AND WAIVER OF LIABILITY

I, the parent or guardian of the registrant, a minor, or the named participant agree that we will both abide by the rules of the Tae E Lee Orleans Taekwondo. Recognizing the possibility of physical injury associated with Tae Kwon Do and in consideration for the program accepting the registrant for its Tae Kwon Do program and activities I, hereby release and hold harmless the Tae E Lee Orleans Taekwondo, their instructors, and associated personnel including the owners of the facilities utilized for the program, against any claim by or on behalf of the registrant as a result of the registrants participation in the program and/or being transported to or from the same, which transportation I hereby authorize.

Please note: participants must supply/purchase their own protective equipment

In the event of an emergency, the student/parents/guardian give permission for emergency first aid to be administered or emergency medical help be called by the representatives of Tae E Lee Orleans Taekwondo, only 1st Aid will be administered and the student/parents/guardians will hold Tae E Lee Orleans Taekwondo harmless for any consequences of first aid and will assume the costs of such treatment. Reasonable attempts will be made to contact the parent/guardian/emergency contact, in the event of failure to do so the student/parent/guardian give permission for Tae E Lee Orleans Taekwondo to act on their behalf in the case of an emergency for the best medical interest of the student.
